Role Description:

This is a full-time on-site role for a Registered Nurse Case Manager at Shanti Hospice and Palliative Care in the East Valley of Phoenix, AZ. The Registered Nurse will be responsible for providing compassionate end-of-life care, admitting, discharging and managing all aspects of care for the patient, collaborating with interdisciplinary teams, and supporting patients and their families.

Qualifications:

Patient assessment, care coordination, and case management skills
Strong communication and interpersonal skills
Ability to work effectively in interdisciplinary teams
Compassion, empathy, and respect for patients and their families
Active RN license in the state of Arizona
BSN degree preferred
Experience in hospice or palliative care is a plus
